How do I enroll?

Enrolling in the registry is easy, and there are two ways to do it:

1. Ask for a registry consent form from your Washington University colorectal surgeon, genetic counselor or gastroenterologist.
2. Call the Colorectal Surgery Office at 314-454-7177 and request that a consent form be mailed to you. Enrollment is not limited to patients of Washington University physicians. Registries have been shown to help reduce the incidence of colorectal cancer and, thus, are an important regional resource. We want to enroll you in our registry regardless of your location or treating physician so that we can play our part in providing educational resources to aid in reduction of cancer.
